Can it promise KDP will accept my file?

No. Only KDP decides. It checks every reason KDP's own help pages give for refusing a paperback file.

Does it upload or change my PDFs?

No. It only reads them.

What if something can't be judged from a PDF?

It says NOT CHECKED and tells you what to look at by hand.
